Mr. Smith is an 88-year-old client who presents from an extended care facility to a primary care office. He reports nausea, vomiting, difficulty urinating for the past 48 hours. You observe visible chills. The client appears lethargic and is intermittently confused. His vital signs are temperature: 101.2, pulse: 103, respiration rate: 30 and B/P: 100/68. His pulse ox reading is 96% on room air. On physical exam suprapubic tenderness and flank pain are noted. His neurological exam reveals an unsteady gait and difficulty hearing and short-term memory deficits. His BMI is 18 and he appears emaciated. A clean catch dipstick urinalysis reveals urine that has a foul odor and the following:
